  2. Press 1 for English
  3. Press 1 for Provider
  4. Enter Tax ID Number
  5. Enter last five of license number (California Providers) or Enter NPI number (Non-California Providers)
  6. Press 2 for Claim Status
  7. Press 1 to enter Subscriber ID or 2 to enter Subscriber Social Security Number
  8. Enter number
    • If you pressed 1, press 1 again if the ID number begins with a W and 2 if it begins with a Y
  9. Enter the Subscribers date of birth
  10. Enter the date of service
  11. Enter the submitted charge
  12. Claim Information Provided
